Help on how to integrate into current site

All I would like to use the control to do is show previews of the image file the user has selected.

There will be several upload fields on a single form. The user will put a single file into each control. Each control should have a name like "image1" "image2" "image3" and so on.

When they upload all the files, I need to pull the name of the file they uploaded from each imageN variable from my php script so that the path and filename can be input into a database.

What would be the best way to accomplish this?
