Any advice on how to set up VB6 so that it recognises GDI+?

Do you mean GDI+ drawing features of Graphics Mill? Or you are looking for GDI+ wrappers?

I mean: what else besides GM do I need to install so that VB6 allows me to use the GDI+ drawing features of GM?
I tried using the GDI+ interface and got no output at all, and I suspect that this is because my win2000 system does not support GDI+. This does not come standard with Win2000.
I then tried to download a GDI+ installation file from microsoft and installed it but in VB6 I cannot find where to set a reference to it.

Is this latter in fact necessary? If not, any tips why the GDI+ may simply do nothing from GM?

You need to put gdiplus.dll in the same folder with GraphicsMill20.dll. No need to add reference, just put this file (you can find it in the Graphics Mill installation folder).
